After the first sexual intercourse the hymen is ruptured and only some remnants exist. Other reasons of hymeneal rapture may be: rape, insertion of tampons, sports (like bicycling, horse riding, gymnastics), accidents, falls, etc. Usually, slight bleeding gives the sign that the hymen has been raptured.

How Hymen repair is done
Hymenoplasty can be performed soon after the rupture or later. When repairing the hymen, we tighten the remaining tears together to a virgin-like status.
Hymen repair is usually performed on an outpatient basis, under local anaesthesia or intravenous sedation, if requested. Dissolvable stiches that do not require removal, are used to close the tear by suturing the remnants together. The procedure is well tolerated and takes between one and two hours. The patient can return to normal everyday activities the following day. Following hymen repair, infection, bruising, swelling, discomfort, etc may appear but your doctor is in position to reduce or confront with these rare complications. Intense exercise should be avoided and hygiene should be a high priority. In most cases, the surgery is undetectable after healing.
